If you are using health insurance, please send us the following info:
- Subscriber’s Name and Date of Birth (This can be a spouse if you are under their plan)
- Copy of the Front & Back of your Insurance Card. You can scan and email it, or just use your cell phone to take a picture and email it.
- The patient’s name and date of birth.
